How to order a radiology report?
We reserve the right to refuse to provide a given service in the following cases:
- niedostarczenie kompletnych informacji przez pacjenta, które są niezbędne do wykonania usługi
- naruszenia przez Usługobiorcę warunków Regulaminu, obowiązującego prawa lub ogólnie przyjętych norm społecznych i moralnych;
- dostarczenie niediagnostycznych obrazów badania TK/ MR lub badań starszych niż 3 miesiące
- dostarczenie nieczytelnych skanów opisów poprzednich badań diagnostycznych;
- zaistnienie uzasadnionego podejrzenia co do zamiaru skorzystania przez pacjenta z usługi niezgodnie z jej przeznaczeniem lub w sposób sprzeczny z obowiązującymi przepisami prawa
In the above-mentioned cases, you will be contacted by the coordinator who will inform you about relevant circumstances. Costs of radiology report shall be reimbursed within 7 days of the date on which you will be contacted by the coordinator.

- Patient chooses to send a file without signing in
- Patient accepts Terms and conditions
- Patient Enters the e-mail address opisy@eurodiagnosis.pl into “E-mail message” box
- Patient enters their e-mail address into “Your e-mail address” box
- Patient encloses scan/exam image in DICOM image format
- Patient receives a notification that the file has been sent to opisy@eurodiagnosis.pl
Should you lack the possibility to submit the file electronically, please contact our helpline at: +48 800 70 70 72.
We reserve the (right) to refuse to provide a given service in the following cases:
- patient’s failure to submit a complete set of information necessary to provide the service
- submission of low-resolution CT scan / MRI exam images or images older than 3 months
- submission of poor quality scanned reports from previous imaging tests
- existence of a reasonable suspicion of patient’s intention to misuse the service or to use it in a manner contrary to applicable laws
- violation of the Terms and conditions, applicable laws or generally accepted social and moral standards by the customer
In the above-mentioned cases, you will be contacted and informed about relevant circumstances. Costs of radiology report shall be reimbursed within 14 days of the date on which you are contacted by the coordinator.
